  1. Purposes and legal basis for processing – Personal data shall be collected for specified, explicit and legitimate purposes and not further processed in a manor that is incompatible with those purposes.

 

  1. What personal data do we hold and why do we hold it? – we need to obtain your basic details to allow us to contact you regarding your care, your appointments. We also hold an email address incase we have any further new services. We hold your name, date of birth, phone number, email address for these purposes. We only hold adequate, relevant and limited to what is necessary to conduct our service.

 

  1. Further recipients of data – we will only share these details if it necessary for the needs of the business or your care. If you have been referred to us via your GP or insurer we may need to write to them and update them on how you are getting on.

 

  1. Data retention – we are required to store data indefinitely, however at anytime you can ask us not to contact you.

 

  1. Your rights – you have the right to access your data at anytime, you can request your data from us at no cost, you have the right not to be contacted for marketing purposes

The new GDRP guidelines outline that if a data subject requests information then this must be provided within a month at no fee. Data subjects are entitled to a description of what personal data we hold, what purposes the data is being held for, the number of years we plan on holding this data.

When a patient requests their information, we will ask them to fill in an information request form. Once this form is signed then we will issue the data to the patient in person or via recorded delivery if the patient is unable to come and collect.

An original file will be kept containing the information recorded by the physiotherapist.

  1. GDRP Subject Access Requests Policy

  1. Rectification

If any errors are reported they will be reviewed by the data processor and or physiotherapist. They may be changed upon request however a note of the original input may need to be kept for medical health records.

 

  1. Erasure

If patients requests for the personal data to be removed, due to health law and the guidelines of the Chartered Society of Physiotherapy we are required to keep records incase of further legislation.

 

  1. Restriction of processing

All patients will be allowed to restrict the way we use their data, if they wish to be removed from marketing lists then we will remove them immediately.

 

  1. Data Portability

If data subjects wish to have the data transferred this request will be met once they have filled in a Data transfer request form. This will be done at no cost to the patient and completed within 1 month of the request.

 

  1. Object to processing

All patients emails are held on a marketing list, patients will be contacted and ask to opt in to the marketing list. Those patients who do not opt in will be removed. Future patients will be asked to opt in at there first face to face contact with the clinic. If they do not wish to be contacted there email address will not be used for marketing. We will retain there email address in case we need to contact them to change an appointment or to discuss their case.

 

  1. Right not to be evaluated on the basis of automated processing

Due to the nature of the business no data subjects are subject to automated processing.
